Event Details
Thank you for your interest in our services for your upcoming event! We are eager to make the children’s experience a magical one. In order to ensure the safety and well-being of the children, there are a few requirements with which we need your help. If the requirements are not adequately met on the day of the event, Sitterwise has the option to release the childcare providers.
Please complete this form, checking each box and filling in details below, then download the PDF and return it to us with your signature and full payment at least one week before the date of service.
Client Will Provide
Please check each item to confirm:
A separate room of adequate size for the free movement of children (at least 25 sq ft per child). Groups of fewer than 4 children should use a regular room. A suite is adequate for up to 8 children. For groups of 9 or more, a carpeted function room is required. Large parties may need to be split between two rooms to provide a separate nursery for babies and toddlers. The room must have a door that can be closed for safety.
For events lasting past 9:00 PM, we require a screen or projector so that children can have “down-time” around their normal bedtime.
For events lasting past 9:00 PM, we require pillows and blankets for the children. Cribs for children under 2 must also be provided (1 crib per 2 babies). Please coordinate with the venue to arrange these items ahead of time.
If the event is scheduled around meal times, appropriate food and drink must be provided for the children. Our caregivers also appreciate your consideration and are very grateful when snacks and drinks are provided for them as well. For babies and toddlers, parents should furnish milk/bottles and appropriate snacks.
A parent sign-in and sign-out sheet will be used for all children. Each parent or guardian must sign their child(ren) in upon arrival and sign them out upon departure. Children will only be released to the adult who signed them in, unless prior written authorization is provided.
